Quarterly Report to Our Communities on Horizon’s Strategic Plan SHOWCASING PROGRESS ACROSS HORIZON Every day, our teams are finding new ways to expand access to primary care, improve patient flow, and strengthen the services our communities rely on. A key goal in our 2025–2030 Strategic Plan, Transforming Health Care Through Innovation, is attaching all New Brunswickers to a primary care provider and expanding the team-based model of care. We have announced 14 teams to date — including four this fiscal year — with more in development. Efforts continue to reduce hospital congestion and improve care transitions. Emergency Department diversion teams have helped avoid more than 640 hospital admissions and saved over 10,000 patient days, while Horizon teams now complete long-term care assessments in five hospitals to reduce delays. Patients are benefiting from improved care delivery, including advances in stroke care, with door-to-CT performance now exceeding the national benchmark of 15 minutes. While challenges persist, particularly as demand for services continues to grow, we are seeing encouraging progress across our strategic priorities and remain confident in our direction.
