Personal Health Records

Due to visitor restrictions associated with COVID-19, in-person visits are not permitted.

As we work to limit the number of people entering Horizon facilities during the COVID-19 pandemic, the Health Records department is closed to visitors.

There are alternate ways to receive personal health record information.

If you require access to your health records, do not visit in-person. Instead, the AUTHORIZATION FOR RELEASE OF PERSONAL HEALTH INFORMATION is available to you online. Click DOWNLOAD FORM.

If you experience issues accessing the form, please see the contact information below, call or email the appropriate area, and a form will be sent to you.

Our Health Information Management team will mail copies of the records to patients requesting information, which will save people from making a trip to the hospital. Please note: health records will not be sent by email.

Low-Priority COVID-19 Test Results for Asymptomatic Travellers

MyHealthNB (myhealth.gnb.ca) is the best and most efficient way to access your COVID-19 test results. MyHealthNB allows you to print an official PDF copy of your COVID-19 test result for the purposes of travel.

If you can’t access your COVID-19 test result from MyHealthNB, you must:

  • Complete the Patient Access Release Form.
  • Submit your completed form to your local Release of Information office by email. Include your name, where and when you had your COVID-19 test, and flight information, such as airline, flight # and departure time and location.  

You will receive a response during regular business hours: 8 a.m. to 4 p.m., Monday to Friday.
