Family Health Teams

Fredericton and upper river valley area

Status: Operational
Patients: Approximately 1,950 patients
Attachment: Accepting new patients from the wait list

The Carleton North Family Health Team was established in May 2025. The team received provincial funding to transform the existing Carleton North Medical Clinic into a family health team. Funding supported infrastructure updates and the addition of several health care professionals, which has already allowed this team to onboard 250 previously unattached patients.

Numerous health professionals have since joined this team, including a respiratory therapist and a social worker, and recruitment is currently underway for several more primary care providers and allied health professionals.

Once these additional health professionals are in place, the team will be equipped to continue towards their goal of onboarding another 800 unattached patients by the end of 2026.

The Fredericton North Family Health Team was Horizon’s first collaborative care team to be announced in April 2025. The team was originally established in 2021 as the Fredericton Northside Community Health Centre and received provincial funding in early 2025 to build new infrastructure and accommodate numerous health care professionals including four primary care providers and 17 additional full-time health care and supporting positions.

The team moved into its new and expanded location on Dec. 8, 2025, and in addition to attaching nearly 1,200 new patients they have also successfully prevented the unattachment of 800 patients.

Horizon received $1.7 million in provincial funding to expand the Nackawic Health Centre, ensuring every unattached resident in the Rural Community of Nackawic-Millville has access to a primary care provider. As part of this investment, Horizon has secured a lease for a secondary clinical space located within the existing Nackawic Health Centre facility. The centre will provide health care services out of both clinic locations and the new area, totalling approximately 3,900 square feet, will help accommodate the team’s ongoing and extensive expansion.

Renovations have already begun to the secondary clinic space and are expected to be completed by fall 2026. The team has recently welcomed several new health professionals including a physician (previously operating in the community as a fee-for-service provider) and two new nurse practitioners (NPs). One NP is already onboarding patients and the other will join the team in July 2026.

Patients: Approximately 11,000

Horizon received $1.6 million in provincial funding to form the Perth-Andover Family Health Team in Southern Victoria. The team was created to support long-term improvements to health care in the region, including stabilizing access to health care as local physicians near retirement, introducing new allied health services and connecting all unattached patients in the region to a primary care provider.

The team began practising in November 2025 and has already made significant progress in addressing the region’s primary care needs. In just a few short months, approximately 500 patients have been successfully attached to this team. This team expects to onboard all remaining local unattached patients from the provincial registry within 12 to 18 months.

Miramichi Area

Status: Operational
Patients: Approximately 5,053 patients
Attachment: Accepting priority referral patients and unattached newborn babies. Will once again begin accepting patients from the wait list in fall 2026.

The Nordin Family Health Team received provincial funding in June 2025 to expand their team and onboard an additional 1,000 patients within the next two years.

Since that time, this team has onboarded approximately 2,400 patients from two retiring physicians and is well on its way to meeting attachment goals.

In the past year, three physicians, three licensed practical nurses, a registered nurse, a pharmacist, a personal care attendant, two medical office assistants, a primary care manager, and an access coordinator have joined the team.

In addition, the team has also integrated mental health services – a licensed counselling therapist-candidate joined in September 2025, and a social worker joined in November 2025.

Recruitment is underway for two vacant nurse practitioner positions, a respiratory therapist, physiotherapist and a registered nurse.

Moncton Area

Status: Operational
Patients: Approximately 550
Attachment: Accepting new patients from the wait list

The Mapleton Family Health Team in Moncton received $2.4 million to expand clinic space, add more health care professionals and onboard unattached patients.

Since launching as a Horizon Family Health Team in October 2025, the nurse-practitioner led team has onboarded more than 500 patients, through system stabilization and providing care for previously unattached. They will continue to onboard patients to reach the goal of attaching 700 patients without a primary care provider by October 2026.

An allied health team hub has been established, supporting both patients of the Mapleton team and other teams in the community. Work to renovate three amalgamated suites is now underway and is expected to be completed over the next eight to 12 months. The additional space will be able to accommodate two additional primary care providers who will be recruited for once the space is ready.

Patients: Approximately 2,263

The Family Health Team at Tantramar Primary Health Care Centre was announced in April 2025.

The team began onboarding patients while assessing options for a larger clinic space that could support the team’s projected growth and targeted patient attachment.

A new clinic space of approximately 10,000 square feet has since been identified, allowing the team to continue expanding primary care services, recruit additional health professionals, and accommodate all patients in the Tantramar region without a primary care provider once the renovation is complete. Renovations to transform the new space into a clinic are underway and expected to be completed within 12 to 16 months.

The team will continue to operate out of their current location (2 Main St.) as renovations are made to the new clinic space (24 York St.).

Saint John Area

Status: Operational
Patients: Approximately 1,460
Attachment: Accepting new patients from the wait list

The Charlotte County Collaborative Wellness Centre (CCCWC) – soon to be the St. Stephen Family Health Team – received $3.6 million in provincial funding to expand access to team-based primary care in the region. Since transitioning to the collaborative care model in February 2025, the CCCWC has expanded from three to 10 health-care professionals, including physicians, nurse practitioners, nurses, a respiratory therapist and a social worker. Since July 2025, this team has already onboarded more than 700 patients and launched an access clinic for unattached patients with non-urgent medical needs, serving more than 2,600 people since opening.

This team currently operates out of Horizon’s Charlotte County Hospital (CCH) and will relocate to a renovated space, located at 210 King St., in early 2027. Renovations to this new space are underway and the team will continue providing care in the existing location (4 Garden St., 3rd level of CCH).

Patients: Approximately 4,159
Attachment: Not accepting new patients from the wait list

The Fundy Health Centre in Eastern Charlotte received a $1.5 million investment to complete a major renovation and add more staff to the existing multidisciplinary team. Renovations are anticipated to take 18 to 24 months and once completed this team is expected to provide primary care services to all patients in the area who are currently without a provider.

The team expanded services for access clinic in January 2026.

Patients: Approximately 482
Attachment: Not accepting new patients

Horizon’s North End Wellness Centre in Saint John received $2.9 million in provincial funding in December 2025 to expand its collaborative care services and increase access to primary care for north end residents under the family health team model.

Funding will support the centre’s continued growth including the identification of a new clinic space and recruitment of additional health care professionals. This expansion will allow the team to attach 10,000 patients.

To support its planning, the team has engaged with community members and partners to further identify the region’s specific health care needs. Most recently, a community engagement session with local service providers was conducted on Feb. 25, 2025, to inform service delivery. Recruitment and patient attachment will occur following new space being ready​.

The Saint Andrews Wellness Centre is a community-based collaborative care clinic, co-run by Horizon and the Town of Saint Andrews. The centre recently received a $1.7 million provincial investment to renovate clinic space, add more health professionals, increase services, and improve access to care.

Renovations to the centre were completed in April 2026 and Horizon opened blood and specimen collection services in May 2026. Come September, three new physicians will join the team to attach more patients and ensure continued access to care close to home.
