Patient Experience Survey We’re listening! We value your voice and want to hear about your experience. We are asking for feedback from patients who have visited a Horizon Emergency Department or have been discharged after staying at least one night in a Horizon hospital or Horizon’s Stan Cassidy Centre for Rehabilitation. This survey will take approximately five minutes to complete and focuses on communication, respect and safety to help us understand your overall care experience. Participation is voluntary and responses are anonymous.
