Cardiac rehabilitation is an education and exercise program that helps people gain information about how to manage their disease through changing their daily health habits. Horizon’s health care professionals such as nurses, physiotherapists, dietitians and pharmacists are available to provide the information you need to recover from a heart attack, other forms of heart disease or surgery to treat heart disease. How is this service provided? This free program runs over a period of generally 12 weeks and involves supervised exercise at a facility or a supervised home exercise program. Classes are held to provide information about heart disease and how to improve risk factors associated with heart disease. How can I access this program? The program is open to people who have heart disease and need help in adapting heart healthy behaviours. Your family physician or cardiologist can refer you to the program. Once the referral is received, a health care professional will contact you to set up an initial visit. Your physician or cardiologist can refer you to the program either before you leave the hospital or after you have been discharged. The referral is sent to a health care provider who will review the information. The health care provider will contact you to set up an appointment. At this first appointment you will meet with a nurse or physiotherapist who will check your heart rate, blood pressure, listen to your heart and lungs, and assess your current level of fitness. Depending upon how fit you are, they will recommend different exercise options for you. The program requires a 12-week commitment from participants. How do I prepare for this program? You should wear comfortable, non-restrictive clothes. If you have specific questions or goals, please share them with your educator. You may need a stress test before the program begins. Education Sessions: Offered weekly or twice a week on topics such as heart healthy eating, medications, quitting smoking, stress management, blood pressure and cholesterol. Exercise Sessions: Weekly or biweekly one hour sessions. Includes circuit training, strengthening and cardio exercises using treadmills, walking, cycling. Blood pressure and heart rate are monitored during the program. Follow-ups: Offered at 3 and 6 months after finishing the program. See your doctor to be referred to the program. What are the benefits of the program? How do I get started? Education sessions: Education sessions are offered on a recurring monthly basis, twice a week for 4 weeks on topics such as understanding heart disease, healthy heart eating, risk factor modification, quitting smoking, medications, stress management, and energy conservation. Supervised exercise sessions: Before starting your exercise program you will have an appointment with the registered nurse to review your medical history and discuss your risk factors. The nurse will discuss your treatment goals and make appointments with cardiac rehab team members to help you manage your disease and make healthy lifestyle changes. An exercise stress test may be required before attending the exercise program. An assessment by the physiotherapist or occupational therapist is done to determine your current fitness level. An exercise plan is then designed specifically for you. Sessions may include using exercise equipment such as treadmills, cycling, and ellipticals. Strengthening exercises and flexibility are also available. Blood pressure and heart rate are monitored during the program. Follow up: A follow-up appointment with cardiac rehab team members is given after finishing the exercise program.
