The goal of the Therapeutics Team is to maximize independence and self-care of people who have challenges caused by: traumatic brain injuries spinal cord injuries complex strokes neuromuscular disorders such as multiple sclerosis and amyotrophic lateral sclerosis (ALS) a wide range of other acquired or inherited neurological conditions Individuals may be seen as inpatients, outpatients or on outreach. They may be seen by a single service or by an interdisciplinary team. The program is designed to meet the individual needs of the clients. Physiotherapists assist a patient in relearning how to stand, move and walk. Who we are Our team includes professionals in: Clinical Dietetics Neuropsychology Nursing Occupational Therapy Physiotherapy Psychology Physiatry (physician specialists in rehabilitation) Recreation Therapy Pastoral Care Social Work Speech-Language Pathology The patient and their family/caregivers are an important part of the team. At SCCR, they are encouraged to participate in goal setting and treatment planning. Services we offer Inpatient services There are 20 inpatient beds available for individuals who need highly specialized services. Inpatients receive daily treatment from a team of healthcare professionals and a physician. There is at least one family conference and each patient’s progress is reviewed at a weekly meeting. A detailed treatment plan is developed and is directed toward achieving the goals of both patient and the team. Plans for discharge begin early and continue throughout the patient’s stay. Often, there are outpatient follow-up appointments and outpatient services with, for example, physiotherapy, occupational therapy or social work. Transitional Living Suites (TLS) These accessible and self-contained living spaces on site at SCCR are designed for patients requiring intensive assessment and/or rehabilitation but who do not require intensive nursing care. Outpatient services Some single service outpatient services are offered in occupational therapy, physiotherapy, psychology, and social work. We also offer limited outpatient service in clinical dietetics, recreation therapy, and speech-language pathology through some of our specialized clinics. Telerehabilitation We strive to use the latest available technologies to provide rehab services at a distance in order to best serve our patients around the province. Specialty Clinics The Therapeutics Team has 3 specialty clinics: Neuromuscular Disease Clinic ALS Coordinated Care Orthotics Clinic Outreach Services Clinical Outreach Therapeutics team staff will consult on specific patients or general disorder areas with professionals around the province, either in person or by videoconference. This is done upon request, or as determined by the team, pending staffing and budget availability. Educational Outreach Therapeutics team staff will provide educational sessions to professionals, stakeholder groups, or the public, either in person or by videoconference. In order to request a session, or to make a suggestion on a presentation topic, please complete the Adult Team Educational Outreach Questionnaire and fax it to the Therapeutics Team at 506-447-4429. Therapeutic Park Atlantic Canada’s first therapeutic park is now open, a place where patients of all ages can practice their independence skills in the fresh air. What to expect Inpatient Services You and your family are important members of the team. We are partners with you in your care. You and those closest to you, are part of the team that will work together to set goals and meet your rehabilitation needs. The team will assess and work on the challenges you may face, and the support you and your family may need. Outpatient Services If you are referred to our outpatient services: You will be contacted with the date and time of your appointment either by telephone or by mail. If your health status changes prior to your appointment, please contact your family physician or the referring physician for guidance and/or consultation. On the day of your appointment: Please arrive at least 15 minutes early. Check in at reception when you arrive, and present your Medicare card. Parking is available in the front of our building for those with a wheelchair accessible pass. Alternate parking is available in the Dr. Everett Chalmers Regional Hospital Parking Lot. Upon check-in, please ask our receptionist for a parking pass. Please bring any related equipment (e.g., wheelchair, walker, braces) to your appointment if applicable. Should you require any assistance, please make arrangements to have someone accompany you. In the case of severe weather conditions, please call ahead to make sure your appointment is still on. How to be referred Individuals must be referred by their family doctor, a specialist, or a Nurse Practitioner. Referrals are reviewed by a pre-admissions committee to ensure they are directed to the appropriate service. In order to be referred to the Adult Program, please have your physician, specialist, or Nurse Practitioner write a consultation request letter and fax it to 506-452-5190, mail it to the address included on the form, or email it to Adultreferrals.SCCR@horizonnb.ca. Please include relevant medical reports and other supporting documentation to the referral. Adult Referral Letter What is the cost of services There is no charge for the services offered by the Therapeutics Team. The cost for these services is covered through the provincial Medicare system, with the exception of Adaptive Driving Services. Advanced technology can be used to assist patients in progressing further to meet their goals. Neuromuscular Disease Clinic An outpatient interdisciplinary clinic for patients with progressive diseases like MS, ALS and Muscular Dystrophy Patients are usually first seen by physiatrist, who then determines other disciplines for clinic. Priority is for patients with conditions that change over time, and for who have difficulty travelling. Occurs on the last Wednesday of the month. ALS Coordinated Care An outpatient clinic for patients with Motor Neuron Disease who are evaluated and treated by Physiatrists, Occupational Therapist and Assistive Technologies specialists. Focus is prospective and interdisciplinary care Patients are also followed closely by Extramural Team who collaborates with SCCR team. Patients are usually seen every 3-4 months Videoconferencing and home visits are arranged as needed. ALS OT Guide Orthotics Clinic The Adult Orthotic Clinic provides an opportunity for the physiatrist, physiotherapist and orthotist to meet together with inpatients and outpatients to review their current orthotics or to prescribe new ones. The team also consults with an orthopedic surgeon if required to discuss the potential for foot and ankle surgery.
