UPSTREAM

What is the UPSTREAM service?

Horizon’s UPSTREAM program screens patients and clients for chronic obstructive pulmonary disease (COPD) and provides diagnosis, education and support. The program is now available at several health centres across Horizon.

UPSTREAM is the INSPIRED Program, but without home visits. This program is currently offered at several Horizon’s community health centres for patients who are able (or would prefer) to leave their homes for appointments.

How is this service provided?

You will meet with a certified respiratory educator who will have you perform a breathing test to assess your COPD, or to confirm a diagnosis.

The educator will work with you / your family to:

  • determine how they can help you meet your goals to improve your breathing,
  • provide education about COPD
  • optimize your medications, in partnership with your physician or nurse practitioner
  • develop an action plan for use when your breathing worsens

If you consent, your action plan can be shared with TeleCare 8-1-1 for after hours support.

How can I access it?

If you are concerned about your breathing, please talk with your family physician or primary health care provider. You may then be referred to us for this service.

How do I prepare for it?

As you prepare for your appointment, you may wish to make a list of what you want to discuss. Many patients find it helpful to write a list of their concerns since writing them all down ensures that they don’t forget anything.
