Horizon’s Charlotte County Collaborative Wellness Centre (CCCWC) provides access to primary health care for the residents of Charlotte County.

Located on the third floor of Horizon’s Charlotte County Hospital, the CCCWC offers a wide-range of integrated services such as vaccinations, injections, well-woman visits and foot care clinics. The health care team delivering these services to the community consists of two nurse practitioners, a licensed practical nurse and administrative support.

Monday to Friday, 8:30 a.m. to 4:30 p.m.
