Horizon’s Charlotte County Collaborative Wellness Centre (CCCWC) provides primary health care services through a collaborative group of health professionals for the residents of Charlotte County.
As part of Horizon’s commitment to enhancing access to primary care, a collaborative health care team at CCCWC is providing services to the community. This team includes a physician, two nurse practitioners, a licensed practical nurse, and administrative support.
Located on the third floor of Horizon’s Charlotte County Hospital, the CCCWC offers a wide-range of integrated services such as vaccinations, injections, well-woman visits and foot care clinics.
Hours of Operation
Monday to Friday, 8:30 a.m. to 4:30 p.m.
