Virtual Care Horizon has been using Virtual Care (also referred to as Telehealth) since 1998. This has helped thousands of patients and clinicians enhance their care. Since March 2020, more than 1,500 clinicians have begun using Horizon’s Virtual Care Platform. Horizon’s community health centres are able to book Virtual Care appointments. Ask your primary care provider for a Virtual Care appointment when booking. Patients can use their own devices to connect with their primary care provider and other clinicians on our Horizon network in a private and secure manner. To access Virtual Care, patients need: Internet access Device (tablet, phone, iPad, computer with camera etc.) that allows video and audio (similar to FaceTime or Skype) Horizon’s Mobile App Patients can use Horizon’s Virtual App (by invitation from their clinician) to securely send pictures and videos for treatment purposes.
