Horizon Health Network and EM/ANB Inc. to continue partnership to increase access to primary care for unattached patients in New Brunswick

(HORIZON/EM ANB Inc.) Horizon Health Network (Horizon) and EM/ANB Inc. will continue their partnership to explore opportunities to expand a collaborative integrated service delivery model that will enhance access to primary care for unattached patients in New Brunswick.

By combining resources, expertise and a mutual commitment to innovation and patient-centred care, the partnership aims to provide a clear roadmap for delivering meaningful improvements in primary care access.

Building on the recent launch of this partnership in the Fredericton area, Horizon and EM/ANB have a shared vision of offering a seamless primary care model that will benefit patients by increasing access points to primary health care services for unattached patients.

The experiences and best practices from the Fredericton zone will be used to guide possible approaches in other regions of the province.

“This partnership allows both organizations to bring our collective strengths to the table, such as Horizon’s extensive experience in managing health human resources and ensuring safe and quality health care services, as well as EM/ANB’s innovative NB Health Link network, which includes a wide range of facilities, technologies, and processes at current and future primary care sites,” said Margaret Melanson, President and CEO, Horizon Health Network. “Our ultimate goal is to create a streamlined and integrated experience for patients across New Brunswick, benefiting those who need care the most.”

Both Horizon and EM/ANB are actively engaged in identifying additional opportunities to involve physicians, nurse practitioners, registered nurses, and other allied health professionals in improving access for unattached patients.

This will help ensure the initiative remains dynamic and responsive to the evolving needs of unattached patients.

As this partnership evolves, Horizon and EM/ANB will continue to evaluate and adapt their strategies, always with the goal of making primary care accessible and seamless for all New Brunswickers.

The integrated model will provide patients with a centralized health record that follows them with each of their appointments, allowing for any follow-up that is needed and providing a true ongoing health care experience.
