The Charlotte County Collaborative Wellness Centre (CCCWC) offers a weekly open access clinic for patients without a primary care provider who are seeking care for non-urgent medical issues. The clinic runs one day a week.

 

Patients can call 506-465-4473 to find out when the clinic will run that week, and to book an appointment.

 

Please note: This is not a walk-in clinic.All visits are by appointment only.

 

The CCCWC is located on the third floor of Horizon’s Charlotte County Hospital.

Horizon’s Charlotte County Collaborative Wellness Centre (CCCWC) provides primary health care services through a collaborative group of health professionals for the residents of Charlotte County.

As part of Horizon’s commitment to enhancing access to primary care, a collaborative health care team at CCCWC is providing services to the community. This team includes nurse practitioners, registered nurses, a licensed practical nurse, a respiratory therapist, social worker and administrative support staff.

Located on the third floor of Horizon’s Charlotte County Hospital, the team at CCCWC offers a wide-range of primary health care services such as:

  • Women and children’s health services
  • Foot care
  • Specimen collection for rostered patients
  • Sexual health services
  • Community outreach (sexual health services in the area’s high schools)
  • Social work
  • Flu immunization clinics
  • Respiratory therapy
