Horizon’s health care professionals such as nurses, physiotherapists, dietitians and pharmacists are available to provide the information you need to recover from a heart attack, other forms of heart disease or surgery to treat heart disease.

How is this service provided?

This program runs over a period of generally 12 weeks and involves supervised exercise at a facility or a supervised home exercise program. The program is free of charge with a valid Canadian Medicare card.

How can I access this program?

The program is open to people who have heart disease and need help in adapting heart healthy behaviours.

Your family physician or cardiologist can refer you to the program. Once the referral is received, a health care professional will contact you to set up an initial visit.
