Centralized Optimization Referral and Evaluation (CORE) Clinics

Horizon has implemented a streamlined referral process for new hip and knee replacement surgery requests. Through this updated process, patients will be referred to a centralized team that will review and triage all cases.

As part of the process, some patients may need to attend one of Horizon’s new Centralized Optimization Referral and Evaluation (CORE) clinics to be evaluated before surgery.

Health care providers can request a referral form by contacting one of the CORE clinics.

Having surgery can be one of the most vulnerable points in a person’s life. Horizon is pleased to offer you these videos to help you or your loved one prepare for your knee or hip replacement.

The purpose of these videos is to help you learn what to expect while you are in hospital, and how to care for yourself after surgery so that you can have the best recovery possible.

Research shows patients receive the best care when they are active participants. We recommend you select a friend or family member to be your partner in care, someone who will be prepared to help you when you are most vulnerable. This might include assisting you with personal hygiene activities after your surgery when you are home.
