Horizon’s Perth-Andover Family Health Team provides primary care and allied health services to Southern Victoria residents. The team brings together five physicians, two nurse practitioners, three registered nurses, two licensed practical nurses, a dietitian, an occupational therapist, a diabetes educator and several administrative staff to form Horizon’s first collaborative care team in this community. Plans are underway to recruit an additional two nurse practitioners, another licensed practical nurse and a social worker. This team operates out of two clinics, conveniently located across the street from one another. This team provides: Primary care, Dietitian services, Occupational therapy, and diabetes education.
