Horizon launches innovative CORE clinics to improve access to orthopedic surgeries February 6, 2026 Centralized system puts patients in control, with goal of reducing wait times for hip and knee replacement surgeries (HORIZON) – Horizon Health Network (Horizon) has launched Centralized Optimization Referral and Evaluation (CORE) clinics in Fredericton, Saint John, Moncton and Miramichi, marking a significant advancement in how patients access hip and knee replacement surgeries across the province. The central intake model represents a shift from traditional referral processes. Instead of being placed on individual surgeon wait lists, patients are now referred to a central location where specialized teams review each case and contact patients directly to offer appointments with the surgeon who has the shortest wait time – whether by zone or individual provider. The final choice remains with the patient. “This is a transformative step forward in delivering patient-centered care,” said Greg Doiron, Horizon’s Vice President of Clinical Operations. “By centralizing our referral process and giving patients more control over their surgical journey, we’re not only improving access to care but ensuring every patient is medically optimized before surgery.” The CORE model is emerging as a best practice across Canada. Provinces including British Columbia, Alberta, Ontario and Nova Scotia have implemented similar systems, with some jurisdictions reporting centralized referral clinics have reduced wait times by 20 to 30 percent. Horizon’s system incorporates evidence-based triage and assessment protocols, where patients with mild to moderate symptoms are guided through alternative pathways tailored to their needs, which may include assessment by physiotherapists, nurse practitioners, or other non-surgical management options. Those accepted for surgery receive medical optimization through the CORE clinics in collaboration with primary care providers. “The CORE clinics ensure that patients who need surgery are able to get it, while those who can benefit from conservative management receive appropriate care,” said Dr. Patricia Bryden, Physician Lead of Horizon’s Surgical Services Program. “This targeted approach means better outcomes for everyone.” Proactive presurgical optimization has been shown to lead to better surgical outcomes, faster recovery times and greater satisfaction for both patients and health care teams. By addressing risk factors before surgery, the CORE clinics help ensure patients are in the best possible condition when they reach the operating room.
