Our Plan

Horizon is working closely with the Government of New Brunswick to establish Family Health Teams that meet the needs of New Brunswickers.

We are prioritizing the communities with the greatest need and are addressing patients without a primary provider who have the greatest health care needs first.

Currently, nine Family Health Teams are in development. At the same time, Horizon’s 46 Community Health Centres are undergoing improvements and transformations to increase access and optimize capacity to serve patients within the Family Health Team model.

Dr. Ravneet Comstock
Physician Program Lead, Primary Care

Dr. Ravneet Comstock is responsible for the development of strategic directions with the local departments of Family and Community Medicine. Dr. Comstock guides the transformation of primary health care and oversees matters related to the service design, quality of medical care, and professional development of its members. Dr. Comstock also collaborates with Dalhousie University’s Faculty of Medicine to effectively integrate clinical and academic responsibilities in alignment with the strategic operational activities of the Faculty and Horizon Health Network.

Existing Community Health Centres Family Health Teams in Development

Existing Community Health Centres are actively providing primary health care services.

 

Family Health Teams in Development are currently in the co-creation process with primary care providers.

 

 

Notable Achievements

Over the next 12 months, the Mapleton Family Health Team, located at 44 Mapleton Road, will onboard 700 patients who were previously without a primary care provider.

Within the next two years, the Nordin Family Health Team will serve an additional 1,000 patients who do not currently have a primary care provider.

Physicians at the existing Carleton North Medical Clinic have joined Horizon’s primary care model, enabling them to serve an additional 800 patients who do not currently have a primary care provider within the next 18 months.

As of April 2025, this clinic serves 1,650 patients. The team is on target to provide primary care services to all registered patients in the Tantramar community who do not currently have a primary care provider by April 2026.

Nurses and physicians have been recruited to the team. A pharmacist has also started and has helped improve access for patients. This clinic will expand in the summer to accommodate an additional 1,600 patients from the Fredericton Northside community.

This clinic has been able to accommodate more than 800 visits from patients who are not attached to the team, providing support for patients who would not otherwise have access to primary care services.

Over the past six months, 3,600 patients have received same-day access to services at this location.

This open-access clinic in St. Stephen offers same-day appointments for non-urgent medical issues as part of a pilot program. Within the first three weeks of operation, 146 patients were seen, providing a timely and effective option for care, especially for patients without a primary care provider.

Primary care social work support services are now available four days a week by appointment to residents of Neguac and Alnwick. Services include therapy and counselling, guidance during life transitions, and assistance in navigating the health system and social or community programs.
