The team is made up of nurse practitioners, a registered nurse, a licensed practical nurse, a social worker and two administrative support people. The team provides primary health care services including: Sexual health services Monthly Pap clinics Diabetes education Pharmacy support Community outreach Hours of Operation Monday to Friday: 8 a.m. to 4 p.m. Closed on weekends and holidays. All visits are by appointment only.
