The Nordin Family Health Team is part of Horizon’s ongoing commitment to improving access to primary care in the Miramichi area. Once fully established, it will provide comprehensive, care within a collaborative, interdisciplinary, team-based setting.

Over the next two years, the team will serve an additional 1,000 patients who currently do not have a primary care provider. The implementation is being phased in to gradually build capacity and services.

This first phase has brought together two physicians who currently serve approximately 3,000 patients, with one nurse practitioner. These providers are in the process of building their patient panels to onboard unattached patients in need of primary care.

A second nurse practitioner, a registered nurse, licensed practical nurse, patient-care attendant and licensed counselling therapist will also be added during this phase to support clinical operations and patient access.

Then the care team will expand to include other allied health professionals, including a physiotherapist, respiratory therapist, and pharmacist. These roles will enhance the team’s ability to offer integrated and coordinated care, addressing both immediate and long-term health needs.

Once fully implemented, the Nordin Family Health Team will operate as a collaborative hub, connecting patients with the right providers at the right time and supporting long-term health and wellness for the community.


Hours of Operation

Monday to Friday: 8 a.m. to 4:30 p.m.

Closed on weekends and holidays.

All visits are by appointment only.

The Nordin Family Health Team is located at the rear of the Peak Performance Health Center building. Follow the Horizon signage for the entrance.
